The last time I went looking for 4.50 x 19 tubes with offset stems, the
only place I found them was:

They were most cooperative and helpful, and the price was reasonable. Be
sure to ask for a copy of their catalog; they carry an amazing range of
out-of-the-ordinary tires and related stuff!!
