One of my rear axle adjusters came off. The nuts and the end cap/stopper. I asked about fuji locks/nylocks but never did anything about it.
Had to order axle adjusters on ebay. The dealers take a few days to get them and it is 1 new part vs 6 new parts. $15 incl shipping vs $20 incl shipping.
Was it a danger? Kind of...the rear axle moved about 1/8 of an inch. Dumping the clutch could have caused a problem--the axle to move more.
The axle nut had been tightened with a torque wrench.

In lieu of changing out the axle adjuster nuts on bikes with those that
have exposed ends, I either wrapped the very end threads with a few turns
of safety wire or put a plastic electrical wire tie around it tightly and
snipped off the excess with fingernail clippers. Either way, should the nut
work loose, it wouldn't disappear.
I've seen some where they drilled a small hole through the very end of the
adjuster and put a tiny cotter pin or hitch pin through the hole to serve
the same purpose.

A short bit of 5/16" fuel hose over the end of the adjuster stud will keep the threads clean and give some protection against the loss of loose nuts.

Might avoid skinned knuckles when working around the adjusters, too!
